“Arthritis” names more than one disease, and the differences decide what actually helps. The kind we see most is wear-driven — osteoarthritis — and it responds to being treated as a specific problem in a specific joint rather than as a fact of getting older.
What arthritis pain feels like
Osteoarthritis has a recognizable rhythm:
- Stiffness first thing in the morning that loosens once you start moving
- Aching that builds through the day in proportion to what you asked of the joint
- Grinding or crunching you can feel, and sometimes hear
- Swelling and warmth after a heavy day, settling with rest
- Joints that have quietly lost some of their range
Hands make it visible earliest. The jar that now needs someone else. Buttons and clasps that take two attempts. A pen held differently than it was five years ago. When weight-bearing joints are involved, the day gets planned around them — which errands combine, whether the trip is worth what tomorrow costs.
What’s actually happening in your joints
Where two bones meet, the ends are capped with articular cartilage — a smooth layer that lets the surfaces glide — inside a sealed capsule lined with a membrane producing lubricating fluid.
In osteoarthritis, that cartilage thins and roughens. Cartilage carries no nerve supply, so the wearing itself isn’t the pain. The pain comes from what responds to it: the lining inflames and produces excess fluid, the bone beneath the worn surface takes more direct load and turns tender, the capsule thickens, and bone spurs form at the margins. The lubricating fluid thins too, so the surfaces glide less easily.
Inflammatory arthritis works differently. In rheumatoid and psoriatic arthritis the immune system attacks the joint lining itself; in gout, crystals deposit inside the joint. Those are systemic diseases belonging with a rheumatologist, whose treatments act on the disease process directly. That is not what happens here. What this practice offers is aimed at the pain those joints produce — alongside rheumatology care, never as a substitute for it.
How we find the source
Dr. Manuel López, a board-certified pain management and rehabilitation physician, works to establish which joint is generating your pain and what type of arthritis is involved. Your workup may include:
- A joint-by-joint exam, noting which motions are lost and which are simply painful
- The distribution pattern — how many joints, which ones, and whether they match on both sides
- Weight-bearing imaging for the knee and hip, since joint space narrows under load
- Referral for blood work or a rheumatology opinion when the pattern suggests an inflammatory cause
- A diagnostic injection, which confirms a suspected joint when relief follows
Imaging and symptoms often disagree, in both directions. The exam establishes whether the findings and your pain describe the same joint.

When to seek care promptly
Seek care immediately if a single joint turns hot, swollen, and severely painful over a short span, especially with fever or chills — an infected joint is an emergency, and it can look at first like a bad flare. Get evaluated promptly for a joint that suddenly can’t bear weight, and for joint pain arriving with unexplained weight loss, rash, or fever.
