If your new patient consultation is already scheduled, filling out your paperwork ahead of time makes the first visit shorter.

The packet is a PDF. Print and complete it at home and bring it with you, or arrive a little early and fill it out here.

What to bring to your first visit

• A photo ID
• Your insurance card
• A list of your current medications
• Any prior imaging or records related to your pain — MRI, CT, X-rays, reports
• Your completed New Patient Packet, if you filled it out at home

The imaging matters more than people expect. Your first visit with Dr. Manuel López, a board-certified pain management and rehabilitation physician, is a diagnostic visit — the more he can see of what has already been documented, the sooner the conversation gets to what is causing your pain.
